Dear student,

In order to participate in language courses organized by your company, I would like to kindly ask you to fill in the registration form below.

The registration form was created to gather information that will allow us to prepare the course in such a way that it fulfils your expectations.

Mark bellow when you CANNOT attend the lessons:

Group courses can take place outside of the fixed working hours, between 7:00 – 9:30 and 16:00 – 20:00. The course can take place in other time only pursuant written approval of the manager. Individual courses can take place anytime during the day.
